Early Alert - Referral Form

Please use this form to notify the STARR Center of any students who are not making satisfactory academic progress, are chronically absent or late, exhibit disruptive behavior, or show other signs of being at-risk academically or personally. Once alerted, the STARR Center will attempt to contact these students and work with them on ways to improve their performance.
